Requirements for shipping medications to the United States
Formula or medical prescription. (Treatment for 3 months)
Document required by Colombian Customs and the FDA in the United States, which will be provided by an EPS or private doctor. Said document, if issued by a private doctor, must be signed and with the Professional Registration number. The recipient is entitled to shipments for a maximum treatment of 3 months.
FDA form
The FDA is the regulator and protector of public health in the United States (Food and Medicine). This form will be completed by the recipient or sender, in this case it will be issued by iishoexpress.com, since at the time of the packages the FDA will review according to the data provided.
Receipt
This will only be requested when the medications are prescribed by a private doctor, which must be submitted in original.
ID photocopy
The recipient's is required.
Passport
The addressee's photocopy will be presented.
Letter of responsibility
Document required by Antinarcotics in Colombia which will be issued by iishoexpress.com, signature and fingerprint of the sender is required.
